MACS is pleased to announce Centennial Pharmacy Services will be providing vaccinations to our school community for all individuals age 12 and older. If you are interested in receiving a vaccine or having your child vaccinated, please register via the link below and complete the screening form. Any person under 18 receiving a vaccination at this clinic MUST have a completed online registration AND a screening form completed by the parent or legal guardian. *Parents/family members can receive the vaccine also. Students will receive a $10.00 WAWA gift card AFTER receiving the 2nd dose of the vaccine.
